This page asks you for details of your child, where they go to school and the childcare you use or would like to use.
If you have more than one child please complete for the first child and you will have a chance to complete additional forms for any additional children. Please only complete for children who are attending school (from Reception up to Year 9).
For the purpose of this survey, wraparound childcare refers to formal childcare (such as out of school club, childminder or school provision) before and/or after the school day. Childcare does not include extra-curricular activities (for example an after school football or dance activity).
